Leaking
Radiator cap did not work on a 2002 Honda Accord. Started leaking and yes cap was on right. Ended up picking up a new cap at the local Honda dealer and worked perfect.

Fixed my coolant return problem on 2004 Odyssey
Before replacing I'd fill up the reservoir and the reservoir would be dry within a couple of drives. Once I replaced my radiator cap the return tank was holding fluid so the replacement was definitely needed. This one is a slightly narrower where the notches are. But didn't cause any issues. Once I got the angle right and got the 2nd notch in place it closed fine.
