Does the storage box have a lid?

No lid. It's just a piece of place cover lay on the box. Really annoying. I don't know how many times my son flip over the box and these little block pieces were EVERYWHERE!!!

Are the blocks made of plastic or wood ?

Wood. Very slick, lacquered wood.

Are the patterns grooved (like a toddler puzzle) or flat?

Flat, and I think that it is a poor design. It's why the pieces all slip around.
